Enter and send application/report matters
- An email address is required for applications and reports. Notifications of screening results will be sent to this email address.
If you make a mistake, we will not be able to contact to you, so please enter it correctly. - Please prepare the necessary documents digitally (file format must be JPG, PNG, or PDF).
*One file can be attached up to 10MB.
*You can also apply and report images taken with a smartphone. When taking images, please be careful about the data capacity, such as reducing the resolution. - The secretariat may contact you regarding the details of your application/report.

Mail by a method that leaves a record
- Please print the application/report documents on A4 paper. Attached documents may be black and white copies, but please make sure the contents are legible.
- Please mail your application/report documents to the address below. We cannot accept applications brought to the Tokyo Metropolitan Government Office or secretariat, sent by any means other than mail, or faxed.
- Please use trackable mail, such as registered mail.
- Please pay the postage costs yourself.
- Once applications and report documents have been accepted, they cannot be returned regardless of whether the issuance decision is made or not. If you submit the original, please be sure to keep a copy for your records. The secretariat may contact you regarding the details of your application/report.
